Correct format for a letter to somebody of unknown sex

What should the correct appellation be for a recipient of unknown sex?

I'm tempted to go with "Dear Sir stroke Madam".

Although I have been reliably told "Madam stroke Sir" is correct for the same reason most online forms now list the gender menu "Female over Male" i.e. they follow alphabetical order.

(And before you ask, I cannot contact the recipient entity to find out the gender of the recipient individual. If I could, I certainly would do this.)

Nah, don't go with that wussy swill. Start with "Dear Sir/Madam,".

No, it's just myself adding variation to further trick people into thinking we're different.

Having said that, letter writing is a dying and beautiful art. Preserve it!

Dear aristocratic barstard / erroneous bitch :D

Is their a written obligation to address someone in a particular manner? As if one were to choose to feel aggravated at the title of whom the letter is addressed to, I would question their integrity now. To become stricken by the ego at one's desire to be referred to by a specific external status, rather than the content of the letter is a perplexity to me.

Although where addressing an authority is concerned, I can see the above point fade in an instant.

I've usually went with
To whom it may consern.

Dear Sir/Ma'am,
I'm unsure if the slash is better than "or", but to me "ma'am" refers more to the sex and less to her age, even though it's more of a military term.
